Employer-Provided Health Insurance Offer and Coverage — Notice of Availability
AMD Senior Purecare LLC (dba AMD Home Care) prepares IRS Form 1095-C for each employee who was a full-time employee for any month of the year. In accordance with IRS Notice 2025-15, we furnish Form 1095-C on request.
You may request a copy of your Form 1095-C for any tax year (2024, 2025, or later) at any time. Please include your full name and the tax year you need.
